Six labeled sections move a reader from the basics to genuine reflection. Section one captures how the book was read (independently, with someone, or as a read-aloud), the format (print, e-book, or audiobook), pages read, and time spent. Usage Notes

The ruled lines throughout are spaced generously enough for a standard ballpoint pen without crowding. Gel pens work in the wider summary and reflection boxes, but a medium-tip is better than a broad nib in the three narrow My Response columns. The sheet can be filled in digitally as a PDF if your child prefers typing over handwriting.

Printing Tips

Single-sided on 20 lb copy paper works fine for one-time use. If you plan to reuse the sheet across many books, laminate a copy and let students fill it in with dry-erase markers, wiping clean after each book. Print a batch of five or ten at once and keep them in a reading folder so one is always ready when a book is finished.
