Every book your student finishes deserves more than a one-line summary. This reading and literature analysis worksheet walks them from plot to theme to personal response, all on one page.
The reading and literature analysis worksheet opens with a Book Information table covering title, author, genre, date started, date finished, location read, and a notes row. Below that, three left-column sections handle Summary, a two-column Main Characters table with character name and description or role, and a Setting block with three guided questions about place, time, and influence. The right column runs five Literature Analysis prompts asking about central theme, textual evidence, character development, author’s purpose, and literary elements such as symbolism and foreshadowing. A full-width Personal Response section at the bottom asks what the student learned and how the book made them feel, and a Favorite Quote bar with a page number field closes the sheet.
This form fits naturally into the Homeschooling Forms collection as a companion to weekly reading logs. Students who read independently can complete it solo, while younger readers can use it as a discussion guide with a parent. The literature analysis column gives two to three lines per question, enough for a focused answer without overwhelming a reluctant writer.

Gel pens or a 0.5 mm rollerball work well throughout since most fields offer two to three lines of moderate width. The Main Characters table has narrow rows, so a fine liner keeps entries readable without crowding. The form can also be filled in digitally as a PDF if your student prefers typing responses.

Cardstock or 24 lb paper holds up better when students write with any pressure in the literature analysis column. Print one copy per book, or run a small batch at the start of a reading unit and store them in a binder with a left margin for hole punching.
