Homeschool PE Activity Log, Monthly Tracker With Goals and Summary

Keeping a homeschool PE activity log turns scattered bike rides and backyard sports into a real, reviewable record your student can actually feel proud of.

The homeschool PE activity log opens with fields for student name, parent or guardian, month and year, and week start date. Below that sits a Goals box prompted by the question ‘What are we working on this month?’ with three write-in lines. The main table runs five columns: Date, Activity, Duration in minutes, Location, and Notes or Skills Practiced, with eight rows for individual entries.

At the bottom, two side-by-side sections round out the month. Notes and Reflections gives five lines to answer ‘How did it go? What did we learn?’ while Monthly Summary tallies total activities, total minutes, favorite activity, and areas to improve. Parents working toward a physical education requirement will find the combination of daily entries and end-of-month totals useful at review time. This page is part of the Homeschooling Forms collection.

Log every workout, sport, and movement session with this homeschool PE activity log. Includes goals, duration tracking, and a monthly summary.

Usage Notes

Fine liners in the 0.3 to 0.5 mm range suit the Date and Duration columns, which are narrow. The wider Notes and Skills Practiced column handles a gel pen comfortably without cramping. Laminate a copy and fill it in with a dry erase marker if you want a reusable weekly version, then transfer monthly totals to a fresh printed sheet.

Printing Tips

A single-sided print on 24 lb paper holds up better than standard copy stock when the sheet gets pulled in and out of a binder weekly. Punch two holes along the left edge before filing so the margin stays intact and nothing clips the activity table.
